The Graduated Approach

How we identify needs, plan support, and help every child thrive
At Stone Cross School, we use the Graduated Approach to ensure that every child receives the right support at the right time. This is a four‑part cycle: Assess, Plan, Do, Review. This helps us to understand each child’s needs and adapt our teaching and provision accordingly.
This approach is continuous, flexible, and centred around the child. It ensures that support is personalised, regularly reviewed, and developed in partnership with families.
1. Assess
We begin by gathering a clear picture of your child’s strengths, needs, and barriers to learning. This may include:
· Observations in class
· Teacher assessments
· Conversations with parents and carers
· Pupil voice
· Advice from external specialists (if appropriate)
We use this information to understand what your child needs to succeed.
2. Plan
Once needs are identified, we work with you to agree on:
· Specific targets for your child
· Strategies and adaptations to support learning
· Any interventions or additional support
· How progress will be measured
Plans are shared with all staff working with your child to ensure consistency.
3. Do
Teachers and support staff put the agreed strategies into action. This may include:
· Adapted teaching
· Targeted small‑group or 1:1 interventions
· Visual supports or scaffolding
· Emotional or social support
· Specialist programmes recommended by external agencies
The class teacher remains responsible for your child’s progress.
4. Review
We speak regularly with families to review:
· Progress towards targets
· What is working well
· What needs to be adapted
· Next steps in support
Reviews typically take place three times a year, but can happen more often if needed. The cycle then begins again, ensuring support remains responsive and effective.

When Additional Support May Be Needed
If, after several cycles of the Graduated Approach, a child continues to experience significant difficulties, we may discuss whether your child needs to move onto an Additional Needs Plan (ANP). If this is still that case following a full cycle on an ANP, a discussion may be had around whether an Education, Health and Care Plan (EHCP) assessment is appropriate. This is always done in close partnership with families.
